Of the original sheet of 100 that was issued, most were used on local mail by the Panama Railroad Co. of these the majority were lost (only 13 used examples are known), however, the error was discovered by Elmer F. Olson, a local philatelist who was able to obtain the remaining supply from the mail clerk. This is one of only 22 mint examples that were discovered.

Apparently one sheet of 100 was sold without anyone noticing the error and, in all likelihood, they were all used for postage. The error was not discovered until a few years after issue and all recorded examples are defective.

This error was caused by the overprint on one sheet being printed too far to the right resulting in the left row not receiving an overprint, when the printer noticed the error, he reinserted the sheet after turning it 180° to correct the problem, he moved it too far which resulted in the second vertical row receiving an additional but inverted impression of the overprint.
